Registration is required for all families requesting child care services. Parents relocating to Charleston AFB who have a need for child care services may pre-register their children upon receipt of their orders.
A copy of the DD Form 2606* and a copy of the member’s orders must be faxed or dropped off to the CDC so that your child(ren) can be posted on the waiting list. (Some of these require the use of IMT viewer or FreeFlow
The effective date is the date received. DD Form 2606* may be secured on line (http://www.e-publishing.af.mil/*) or picked up at any Child Development Center, Family Child Care or Youth Program.
Admission requirements
include completion of:
- AF Form 1181 (Air Force Youth Flight Program Patron Registration) used to document personal data, immunizations, medical information, emergency contacts and special requirements
- DD Form 2652 (Application for Department of Defense Child Care Fees) used to calculate total family income
- CDC Contract
- USDA Food Program Application
Parents must present a current and up-to-date copy of their Leave and Earning Statements (both parents if applicable) for calculating fees. Failure to submit financial information will result in placement in the highest fee category. All forms must be updated annually or when a change takes place.
Other pertinent information such as Air Force Instruction, policy letters, operating instructions etc. is available at the front desk.
Each patron and child(ren) must attend an orientation before starting care at the CDC. The Charleston AFB CDC enrolls all children of Active Duty and DoD Civilians regardless of race, culture, sex, religion, national origin, ancestry or disability.
The Charleston AFB Child Development Center accommodates all children except those whose needs require extreme modifications beyond the capability of the facility and its resources. |
